I have written up the two following letter templates as promised. Suggestions for modifications more than welcomed - I kept them as basic as possible.
Letter to person that provided service
Dear [Contact Person],
On [date], I paid for from you at [location and other details of the transaction].
I am disappointed because reflecting on the time with you, I now feel that you [were misleading/ were untruthful/ used undue force] when obtaining payment from me. I feel this was because [insert reasons here – aim to not be personal, but as factual as possible.]
To resolve the problem I would appreciate your [state the action you require e.g. refund]. Enclosed is a copy of the [receipt, contract, photocopy of reading etc.].
I look forward to hearing from you and to a resolution of this problem. I will wait for [set a time limit] before seeking help from [Trading Standards, consumer group, solicitor]. Please contact me at the above address or by phone [give numbers].

Letter to trading standards
Dear [Contact Person/ Sir or Madam],
On [date], [I/a vulnerable member of my family] paid for a [reading/session with] [name of person/company] at [location and address.]
I am writing to complain that I feel this service was in breach of Consumer Protection Directives. The reason that I feel the service I purchased was in breach of the CPR’s is because [I/they felt pressured into making payment despite not being satisfied/ I/they feel that I/they was/were misled with the service I/they was/were provided/ I/they feel that I/they was/were of a vulnerable state of mind at the time/ enter own reason] and therefore made the purchase whereas I/they probably wouldn’t have in any other circumstances.
To resolve the problem I require that an investigation is conducted into [name of person/company] and the services that they provide due to the way that they are in breach of CPR’s. I have enclosed [letters to/from service provider, transcript of reading/ advertising for psychic/medium that might be misleading.]
